Licensed Backflow Assembly Testers that perform field tests of Backflow Prevention Assemblies within the City of Coeur d’Alene Water Department’s jurisdiction are required to be registered with the City of Coeur d’Alene Water Department.
The City of Coeur d’Alene Water Department requires all State of Idaho licensed Backflow Assembly Testers wanting to perform field tests of Backflow Prevention Assemblies within the City of Coeur d’Alene Water Department’s jurisdiction to submit a completed Backflow Assembly Tester Registration form to the City of Coeur d’Alene Water Department’s Office of Cross Connection Control annually.
Applicants will be listed in the order that COMPLETED forms and required documents are received by this office. Be certain the Backflow Assembly Tester Registration form is filled out completely, accurately, and legibly, accompanied with copies of your State of Idaho Backflow Assembly Tester wallet card, the Certificate of Accuracy for the field kit(s) that will be used to perform field tests, and a signed copy of the Backflow Assembly Tester Conduct Agreement.
It is the responsibility of each Backflow Assembly Tester to submit current proof of valid licensure and field test kit Certificate of Accuracy information as they are renewed throughout the year.
Backflow Assembly Testers working under contract with Fire Sprinkler companies must provide documentation showing proof of employment with said Company.
Completed Backflow Assembly Test Reports may be submitted electronically at [email protected]. Completed Backflow Assembly Test Report forms may also be submitted via fax at 208-769-2336, USPS, or be hand delivered to the City of Coeur d’Alene Water Department Administrative Office at 3145 N. Howard St. Coeur d’Alene, ID 83815 between 8:00 am and 5:00 pm Monday through Friday.
